Candidate's Declaration
1. Candidate's Declaration Regarding Original Documents:
a. I am aware that the details provided by me in any document submitted and/or to be submitted by me in the future, to Ono Academic College and its authorities are complete, correct and accurate and were obtained by me in a proper and legal manner.
b. I am aware that I may be required from time to time to sign additional and/or supplementary forms and/or documents, including electronic documents and I declare that these documents, too, will be true to the original.
c. In the event that it is discovered that the documents do not match the original and/or are not original, then all responsibility for this, will l fall on the declarant and in all this, civil, and/or tortious, as well as criminal responsibility to Ono Academic College and its authorities and to any third party.
d. With this signature, I confirm that I am aware of procedures and requirements of Ono Academic College and that the submission of documents that do not match the original will result in the immediate and full termination of studies, as well as the cancellation of eligibility for a degree, even if full tuition has been paid by me.
e. I declare that I have original documents in my possession which, for which and based on their existence, I was given the opportunity to register for studies. For following are documents: Eligibility for an Israeli matriculation certificate and/or a high school diploma from abroad / Psychometric test score / SAT / ATK / Completion certificate of a preparatory program / Eligibility for a degree / Academic degree grade transcript / Equivalency to matriculation and/or degree. And additional education documents.
f. I hereby give my consent to Ono Academic College to receive from the Ministry of Education or anyone on its behalf authorized by it my matriculation grades for the purpose of examining my application to be accepted for studies in it, and that it is subject to data retention measures, as agreed between Ono Academic College and the Ministry of Education, and subject to Ono Academic College's commitment that my grades will only be used for the purpose of my admission to studies and that all necessary measures will be taken by it to keep the grades with it and to prevent their transfer to others.
2. I am aware that the registration fees are not refundable to candidates who were not accepted or who canceled their registration for any reason, and also that my registration is valid for the term of study for which I registered only.
3. I undertake to comply with all the regulations and instructions of Ono Academic College and its authorities, which exist today and which will be determined during my studies, and to pay the tuition fees that will be determined for each year of study, in accordance with the instructions of Ono Academic College and in this regard including the provisions of the tuition fee regulations of Ono Academic College, as they will be enforced each year, and all this subject to the regulations of Ono Academic College.
4. I am aware that the regulations and instructions of the Ono Academic College and its authorities are subject to changes by authorized bodies and any change that will be made in the future and that relates to me will also be obligatory on me and this without any responsibility or liability being imposed on Ono Academic College in this regard.
5. I hereby give my consent to Ono Academic College to receive / transfer to the Ministry of Immigration and Absorption or anyone on its behalf authorized by it my details for the purpose of examining my eligibility for scholarships from the Ministry of Immigration and Absorption and for participation in various immigrant programs.
6. I am responsible for all property of Ono Academic College that comes into my possession such as: books, writings, devices, other items and I will notify Ono Academic College immediately of any damage to such property, or loss thereof and I will pay Ono Academic College any applicable payment and any other compensation due to a delay in the return of the above-mentioned property due to any damage or loss caused to it.
7. For Israeli citizens - I am aware that in accordance with the National Insurance regulations, my personal details will be transferred to the National Insurance Institute.
