I hereby declare: 

- Demonstrate seriousness, perseverance, and commitment to the mentoring process.

- Record the mentoring hours on a designated form after each session. 

- Notify your mentor of a canceled meeting as soon as possible.

- If I do not give at least 5 hours' notice of a cancellation, the cancellation will count as one mentoring hour. If there are additional cancellations, the mentoring program will be suspended until the mentoring coordinator investigates the matter. 

- I will not continue the mentoring beyond the hours allocated to me. If I am interested in continuing with another series of sessions, I will inform the project coordinator before the mentoring ends, in order to explore this possibility in advance. 

- Pay for the tutoring based on the total number of hours reported. The cost of tutoring is 20 NIS per hour.

- There will be no participation fee for group mentoring. I am aware that if I do not meet these conditions, the matter will be reviewed by the project coordinator, and the mentoring may be terminated.
