Person- and community-oriented rehabilitation coordination
About the course
The work of the rehabilitation coordinator constitutes a significant aspect of the rehabilitation and recovery processes in mental health and includes many and varied activities in connection with the person, the community and other factors. The rehabilitation coordinator is required to have a great deal of knowledge and diverse skills in order to help the person navigate the rehabilitation process in a unique and beneficial way for them.
The course will teach various aspects of the role and functional identity of the rehabilitation coordinator, with reference to models in treatment coordination, the rehabilitation relationship and relationships, skills in developing motivational discourse and the person's readiness for rehabilitation, and skills in professional writing, as well as aspects of mediation and self-advocacy, building person-centered rehabilitation processes, the community context, and more.
The course is intended for professionals involved in rehabilitation coordination who are interested in expanding their knowledge, skills, and tools in their field of work and will assist in providing tools for using knowledge and skills as a basis for promoting the processes of accompaniment, integration, and recovery of people receiving services in various rehabilitation frameworks.
In accordance with Ministry of Health procedures, this course is a requirement for employment in the community mental health rehabilitation system. The rehabilitation coordinator must complete the course designated for his position within 3 years of taking up the position.
Course objectives
The student will delve deeper into the role, personal, and professional identity of the rehabilitation coordinator, and will learn practical tools and skills for assessing, mapping, diagnosing, and developing a person's readiness for rehabilitation and their ability to define personal goals.
The student will be exposed to innovative models in care coordination.
The student will be familiar with at least three intervention tools that are appropriate for the concept of recovery and rehabilitation work, and will experience working with at least one of them.
The student will become familiar with the principles of community integration and its implications for daily work practice.
Course structure
The course includes 15 sessions, each lasting 6 academic hours, with a training workshop in two groups taking place in the last two hours.
Participants
The course is intended for professionals, supported employment coordinators, mentoring and support coordinators, and all professional workers involved in coordinating rehabilitation/treatment in various rehabilitation frameworks.
Course requirements
Attendance is required for 90% of classes.
Active participation in discussions, the tutorial group, and classroom exercises.
Submit assignments and exercises as required.
Participation in an hour and a half training workshop in two groups, including submitting and processing an event.

A professional worker or supported employment coordinator, in one of the rehabilitation frameworks in the community.
